How does the peg removal system work for off-loading?
The insole is constructed with a grid of removable cylindrical pegs across the forefoot area. The clinician identifies the location of the wound or ulceration on the patient's foot, then removes the pegs directly beneath that area using a tool or by hand. With the pegs removed, there is no material supporting that specific zone of the foot, which eliminates direct contact pressure at the wound site during weight-bearing. The surrounding pegs continue to support the rest of the foot, and the stabilizer board prevents the adjacent pegs from collapsing into the void left by the removed pegs.
What is Poron and why is it used as the cover material?
Poron is a high-performance cellular urethane foam used in medical and orthotic applications for its consistent cushioning, pressure distribution, and durability properties. In the Peg-Assist insole, the Poron cover serves a specific clinical purpose: it prevents ring edema and edge abrasion, which are complications that can occur when a wound is off-loaded using a cutout or void in a standard insole. Without a cover material, the edges of the void can create a pressure ring around the wound that impedes healing. The Poron cover bridges the void smoothly, eliminating this edge effect while still allowing the underlying peg removal to reduce direct pressure at the wound site.
