What does the metatarsal shank do?
The semi-rigid metatarsal shank is a stiffened insert built into the sole of the shoe that runs under the metatarsal bones of the forefoot. It limits flexion at the metatarsophalangeal (MP) joints during walking, preventing the forefoot from bending with each step. This is critical after foot surgery — particularly osteotomies and toe procedures — where bending the forefoot can disrupt healing bone, sutures, or soft tissue repair. The shank allows the patient to walk with a controlled, protected gait without requiring a full cast or rigid boot.

The Darco Softie Surgical Shoe is a lower-profile, open-toe post-op shoe designed primarily for forefoot procedures where the toes need to be accessible for dressing changes and where a lightweight, breathable option is preferred. The Hi All Purpose Boot is a closed-toe boot that accommodates bulky dressings and fiberglass casts, providing more comprehensive protection for the entire foot and toes. The Softie is typically used for bunionectomies, hammertoe corrections, and similar forefoot procedures; the Hi Boot is used when full toe and dressing protection is required.
How do I select the correct size?
Select your size based on your pre-surgery shoe size using the size guide above. The Softie is designed to accommodate some swelling, so your normal shoe size is the appropriate starting point. If you anticipate significant post-operative swelling, sizing up one size may provide a more comfortable fit during the early recovery period. Men’s and women’s sizes are separate — select the correct gender column. Note that women’s X-Large is not available in this model.
How do I put on the shoe with limited mobility after surgery?
The one-hand hook and loop closure is specifically designed for patients who cannot bend down easily or who have limited hand strength after surgery. Open the closure fully before putting the shoe on. Slide the foot in gently with the heel seated at the back of the shoe — avoid forcing the foot if swelling is significant. Fasten the closure snugly but not tightly over the dressing or bandage. If dressings are present, take care not to disturb them. A long-handled shoehorn can help guide the heel into the shoe without bending.
